What is the Online Claim Form?

The Online Claim form is a secure and convenient way for you to electronically submit your claim directly to the Ohio Lottery Claims department.

What do I need to use the Online Claim Form?

You will need your signed winning ticket, the signed Pay-to-Bearer and/or the File Claim Receipt and a Valid ID.  The File Claim receipt must be obtained when your winning ticket is validated at any Lottery Retailer.  You will also need a MyLotto Rewards® account. A Pay-to-Bearer is generated for prizes $600 to $5,000 and the File Claim Receipt is generated for prizes greater than $5,000.
 

Where do I find the ticket number to be entered into the Online Claim Form?

  • From any Ohio Lottery Scratch-Off Ticket, type the TICKET NUMBER from the back of the ticket, followed by the 10-digit VERIFICATION NUMBER from the front of the ticket located on under the latex (ignore any spaces or dashes).

  • From your Ohio Lottery printed ticket, type the 35-digit serial number located on the bottom of your ticket.

What file types are accepted when I upload the images of my tickets and ID?

The following file types are accepted when uploading your images:
  • .png
  • .pdf
  • .jpeg
  • .jpg

What is a valid form of ID?

The following forms of ID are acceptable:
  • Valid Driver’s license (Any State)
  • Valid State ID Card (Any State)
  • Military ID
  • Valid Passport
  • Gun Permit
  • Pilot’s License
  • Human Services ID
  • Transit ID (Senior Citizens Only)
  • Bank Customer

I already have a MyLotto Rewards® account.  Do I need to create a separate account for the Online Claim Form?

No, you can use your current MyLotto Rewards® Account. Once you have logged in using your MyLotto Rewards username and password, you will be asked to provide a few additional pieces of information and may need to verify your current information to submit the form.
 

Will my address be changed in MyLotto Rewards if I change it in the Claim Form?

No. If you wish to use a different address in the Claim Form, it will not impact the address you entered for MyLotto Rewards.
 

Why do I need to provide my Social Security number? Is it secure?

Your Social Security number will be used to verify your identity. The Lottery takes data security very seriously and all information provided will be treated as confidential information.
 

Why did I receive an error message when I tried to enter in my legal information (Legal first name, last name and SSN)?

Please make sure you are entering your exact information as it appears on your Social Security card.  For example, if your name is “Michael,”  you need to enter “Michael” and not “Mike.” Make sure there are no extra spaces being added.  If you have recently changed your name, make sure you are using the name that has been filed with the Social Security administration.
 

How long does it take to receive my winnings?

Your winnings will be mailed to you within 30 days.
 

What wins cannot be processed through the Claim Form?

The Claim Form is not available for Mega Millions, Powerball, and Classic Lotto Jackpot wins, Annuity Prizes and Vouchers.  You can contact a regional office  to set up an appointment for these types of claims.
 

I didn’t receive the full amount that I won, where did my money go?

In accordance with ORC 3770.071, 3770.073 Lottery prizes may be reduced by monies owed to the Ohio Department of Job and Family Services or the Attorney General’s Office, if applicable. If your completed deposit shows an Intercept Amount, your prize was deducted for either child support, spousal support or state debt owed via the Ohio Lottery’s Prize Intercept Program. Please review your email for a full explanation. If you have questions, please contact the Ohio Lottery Claims Department at 1-800-686-4208.

If you believe that there were funds withheld in error, please contact these agencies directly at ODJFS at 1-800-686-1556 or the Attorney General’s Office at 1-888-246-0688.
 

Can I cancel my Online Claim Form submission?

No, once you select SUBMIT CLAIM, your submission cannot be cancelled.
 

Can I submit more than one ticket at the same time?

No, you can only submit one ticket at a time.
