Instructions for VLT Employee Gaming Application forms

Employee VLT Gaming License Applicant – The employee is to complete the following forms depending on the type of gaming license the employee is applying for.  The Principals listed on the VLT Gaming application, if determined by the Lottery, may be required to obtain a Key Gaming or Gaming license.
Key Gaming License Applicant – Includes managers, supervisors or other employees who have authority to develop or administer policy, long-range plans, or make discretionary decisions, regulating gaming operations and managers, supervisors or other employees responsible for major areas of the property, even if no gaming is conducted in the area.  This rule is intended to include General Managers but not all managers.

Key Gaming License Applicant with Reciprocity - Reciprocity will be given to Key Gaming applicants that hold a current Key Gaming license from the following states: Colorado, Iowa, Mississippi, Nevada, New Jersey, New York, Pennsylvania, and West Virginia.

Key Gaming Applicant Holding a Current Ohio Casino Control Commission Key Gaming License - Reciprocity will be given to Key Gaming applicants that hold a current Key Gaming license from the Ohio Casino Control Commission.

Gaming License Applicant – Means any employee employed in the operation of a facility whose duties primarily involve maintenance or operation of gaming activity or equipment or who comes in contact with gaming related cash, token, credit vouchers, vouchers, or other financial instruments or who performs security and surveillance activities, but who are not key gaming employees.

Temporary License Applicant – May be granted to an applicant for a Key Gaming or Gaming Employee license until appropriate processing is completed. Temporary Licenses may also be granted to an employee of a video lottery sales agent who is performing short term duties in a facility.  A temporary license may be valid for a term determined by the director.
