COLUMBUS BLUE JACKETS
2nd Period 2nd Chance Promotion
CONDITIONAL PRIZE CONTEST
10/7/11 through 4/7/12 (Columbus Blue Jackets Home Games)
1. The Columbus Blue Jackets (“Blue Jackets”) will conduct the Contest, sponsored by the Ohio Lottery on the dates listed above for a chance to win the following Prizes:
- An autographed jersey, or an autographed hockey stick, or a $50 gift card from the Blue Jackets’ team store; and
- Grand Prize: $1,000.00 cash
2. TO ENTER: NO PURCHASE NECESSARY. Contestants shall drop their non-winning Instant tickets in the Register-To-Win boxes located at the FS Ohio Blue Line and at such other location within Nationwide Arena as Blue Jackets may, in its sole discretion, so designate, between October 7, 2011 and April 7, 2012 (the “Contest Period”). Contestants must write their full name, seat location and cell phone number on the back of each such entry. If any selected prize winner cannot be directly contacted at the game prior to the second period to confirm his/her selection, an alternate prize winner will be selected by the beginning of the second period of the game. Winners must be present at the game to claim the prize and win. No mechanically reproduced or photocopied entries permitted. The Columbus Blue Jackets, the Ohio Lottery Commission and their respective agents, employees, affiliates and subsidiaries are not responsible for illegible, lost, late, damaged, or incomplete entries. Contestants relinquish all rights to the materials submitted in connection with this Contest Materials submitted will be retained by the Blue Jackets for a period of time deemed necessary by the Sponsors.
3. RANDOM DRAWING: Prize Winner will be selected in a random drawing from among all eligible entries received. Prize Winner will be notified on the scoreboard at Nationwide Arena during the first intermission (between the first and second periods) of each Blue Jackets home game and Winner shall also be notified by cell phone. Odds of winning will depend upon the number of eligible entries received.
4. GRAND PRIZE: If the Blue Jackets score a goal during the final sixty (60) seconds of the second (2nd) period, Prize Winner for that game will also win the Grand Prize of $1,000.00 cash. In such event, Grand Prize Winner’s name will be displayed on the scoreboard at Nationwide Arena during the third period of that Blue Jackets home game, and Grand Prize Winner will also be notified by cell phone.
5. GENERAL RULES: The contest is open to legal residents of the United States age 18 and older. Employees of Sponsors (the Columbus Blue Jackets and the Ohio Lottery Commission), their respective agents, affiliates, subsidiaries, advertising and promotion agencies, suppliers, or any blood relative or spouse of such an Officer or Employee living as a member of the Officer’s or Employee’s household are not eligible to enter this promotion. Void where prohibited by law. Any federal, state and/or local taxes are the sole responsibility of Prize Winner. All federal, state and local laws and regulations apply. Acceptance of a Prize constitutes permission to the Sponsors and their agencies to use the Prize Winner's name and/or likeness for purposes of advertising and trade without further authorization or compensation, unless prohibited by law. Further, by accepting a Prize, Prize Winner agrees to hold Sponsors, their respective directors, officers, employees, agents and assigns, absolutely harmless from and against any and all claims and/or liability arising directly or proximately out of Prize Winner’s participation in this promotion and/or the receipt, ownership and/or use of the Prize(s). Prize Winner assumes all liability for any and all injuries and/or damages caused, or claimed to be or have been caused, by Prize Winner’s participation in this promotion and/or his/her use or redemption of any Prize. By participating in this promotion, all entrants agree to be absolutely bound by these Official Rules. This event is conducted solely by the Columbus Blue Jackets with the sponsorship of the Ohio Lottery Commission and is not in any manner an event operated by or pursuant to the rules of the Ohio Lottery Commission.
6. The Ohio Lottery Commission shall not be liable for any direct or indirect damages and/or claims arising from promotions or Columbus Blue Jackets’ conduct of any promotions and/or prizes associated with such promotion(s).
7. If a Columbus Blue Jackets player scores an official goal with sixty (60) seconds or less remaining on the clock during the second period of any Blue Jackets game during the Contest Period , the Prize Winner for that game will win the Grand Prize, subject to verification. A Grand Prize winning goal must occur during the final sixty (60) seconds in the second (2nd) period, and must be an official goal scored by the Columbus Blue Jackets. Any goal scored at any other time during the game, during any other period, or by the opposing team will not be considered a Grand Prize winning goal and no prize will be awarded for any such goal. If an official goal is not scored at during the final sixty (60) seconds of the second (2nd) period by the Columbus Blue Jackets, no Grand Prize will be awarded for that game.
8. The following requirements will apply for any goal scored to be eligible for the Grand Prize award:
- For the purposes of this promotion an official goal must be a goal that is scored by a member of the Columbus Blue Jackets. A goal credited to the Columbus Blue Jackets that occurs as a result of an opposing player erroneously shooting the puck directly into his own goal will not be acknowledged as an official goal for this promotion, and no Grand Prize will be awarded based on such goal.
- A goal scored by the Columbus Blue Jackets during a period of time in which the opposing team has removed their goalkeeper from the game for strategic purposes will not be acknowledged as an official goal for this promotion, and no Grand Prize will be awarded based on such goal.
- No contestant may be selected to participate in this event on more than one occasion.
9. The Grand Prize will be awarded to the Grand Prize Winner within five (5) business days of the date of the Blue Jackets game in which the Grand Prize was won.
10. Persons who have won another contest or promotion of any kind from the Columbus Blue Jackets within 30 calendar days before the date of participation in the Contest are ineligible to win.
11. The Grand Prize winner shall be required to sign a release form before the Grand Prize will be awarded and will be responsible for any and all applicable federal, state and local taxes. The Columbus Blue Jackets is responsible for sending the winner of any prize(s) over $599 a federal tax form.
12. By accepting a Grand Prize, Grand Prize winner agrees to allow American Hole ‘N One, the Columbus Blue Jackets, and the Ohio Lottery to broadcast, publish, disseminate and use his or her name, city of residence, photograph, voice and/or likeness for advertising and/or promotion without further compensation, except where prohibited by law.
13. There is no purchase necessary to enter or win the Contest. Void where prohibited.
14. There will be no substitution for Prizes. Participation in the Contest and Prizes awarded are non-transferable. Failure to collect a Prize constitutes forfeiture of the Prize. No forfeited Prize Winner will be eligible to win the Grand Prize.
15. Ohio Lottery and Columbus Blue Jackets reserve the right to make rule or contest changes without notice at any time. Ohio Lottery and Columbus Blue Jackets also reserve the right to change the aforementioned Contest dates without notice.
16. The decision of the Columbus Blue Jackets management in the interpretation of these Rules is final.
17. By participating in the Contest, all participants agree to be bound by the decisions of the judges and these Official Rules.
18. If any winner is found in violation of these rules he/she may be required to forfeit his/her prize.
19. Ohio Lottery, Blue Jackets, their representatives, agencies and employees, are not responsible for any changes or difficulties in travel or Prize delivery.
